Radiographic Report
Radiographic interpretation: The radiographs confirmed generalized cardiomegaly (VHS = 13) and mild left atrial enlargement (VLAS = 2.7). There are no pulmonary infiltrates identified.
Radiographic diagnosis: Findings consistent with suspected mitral regurgitation without evidence of congestive heart failure.
Clinical interpretation/additional case information: Based on radiographic measurements (VHS and VLAS) and the lack of evidence of congestive heart failure, this dog would be considered Stage B2, likely MMVD. An echocardiogram is recommended to confirm the diagnosis of MMVD, but if declined, a VHS of greater than or equal to 11.5 can be used to support a diagnosis of Stage B2 MMVD in dogs with moderately loud murmurs (greater than or equal to 3/6) characteristic of mitral regurgitation (according to the 2019 ACVIM MMVD consensus statement). This dog has a moderately loud left apical systolic murmur (grade 4/6) consistent with mitral regurgitation and the VHS is 13.0. Based on these findings the CEG and the 2019 ACVIM MMVD consensus statement recommend initiation of pimobendan. The recommended dose is 0.25-0.3 mg/kg twice a day by mouth.
Clinical History
Signalment: 8 year old FS Cavalier King Charles Spaniel (CKCS)
Clinical history: Patient was presented for annual wellness examination and a grade 4/6 left apical systolic murmur characteristic of mitral regurgitation was auscultated. According to the owner the dog is apparently healthy. An echocardiogram was recommended and declined. Radiographs were then performed.
