Radiographic interpretation: The radiographs confirmed suggest mild generalized cardiomegaly (VHS = 11.0) and moderate left atrial enlargement (VLAS = 2.6) with no evidence of pulmonary edema.

Clinical interpretation/additional case information: According to the 2019 ACVIM consensus statement, in the absence of an echocardiographic diagnosis, dogs with a moderately loud systolic murmur consistent with MMVD can be considered Stage B2 if the VHS is greater than or equal to 11.5 or the VLAS is greater than or equal to 3.  In Ballard’s case, the VHS and VLAS are increased relative to normal, however they are not sufficiently enlarged to establish a diagnosis of Stage B2 MMVD at this time. An echocardiogram can again be recommended, however in the absence of radiographic confirmation of Stage B2, no medication is recommended at this time. Recheck radiographs to assess for the presence of progressive cardiomegaly can be performed in 6-8 months.  If at that time the VHS is greater than or equal to 11.5 or the VLAS is greater than or equal to 3, a diagnosis of Stage B2 MMVD can be made and pimobendan can be initiated. Note: According to the 2019 ACVIM Consensus statement, in the absence of an echocardiographic diagnosis of Stage B2 MMVD, exceeding the above VHS or VLAS limits or showing evidence of progressive cardiomegaly (increase in VHS of greater than or equal to 0.08/month or 0.5 per 6 months) are reasonable cut-offs to establish a diagnosis of Stage B2 and lead to the recommendation for initiation of pimobendan.

Clinical History

Signalment: 10 year old MC mix breed dog (7 kg)

Clinical history: Ballard was presented for an annual wellness examination. According to the owner, Ballard is apparently healthy. Physical exam revealed a grade 3/6 left apical systolic murmur characteristic of mitral regurgitation, consistent with a diagnosis of myxomatous mitral valve disease (MMVD). An echocardiogram was recommended (according to the 2019 ACVIM consensus statement) to further characterize the MMVD as Stage B1 or Stage B2 and was declined at this time.
