Radiographic Report
Radiographic interpretation: The radiographs confirmed severe, generalized cardiomegaly (VHS = 12.1) and moderate to severe left atrial enlargement (VLAS = 2.6) with no evidence of pulmonary edema.
Clinical interpretation/additional case information: According to the 2019 ACVIM consensus statement, dogs can be considered Stage B2 based on radiographs alone (without an echocardiogram) if the VHS is greater than or equal to 11.5 or the VLAS is greater than 3. Pimobendan therapy is recommended for these patients.
Clinical History
Signalment: 13 year old MC Maltese
Clinical history: Patient was presented to primary care veterinarian to discuss a dental procedure. Blake has been historically healthy, but has been slowing down over the past year. Physical exam revealed a normal respiratory rate (24 breaths per minute) and a grade 5/6 left apical systolic murmur characteristic of mitral regurgitation, consistent with a diagnosis of myxomatous mitral valve disease (MMVD). An echocardiogram was recommended (according to the 2019 ACVIM consensus statement) to further characterize the MMVD as Stage B1 or Stage B2 and was declined at this time.
