Radiographic Report
Radiographic interpretation: The radiographs confirmed generalized cardiomegaly (VHS of 11.5) and moderate left atrial enlargement (VLAS = 2.8). There is no evidence of pulmonary infiltrates suggestive of pulmonary edema and no pulmonary venous distention.
Clinical interpretation/Additional information This dog’s signalment and physical exam findings combined with the measured VHS and VLAS are consistent with Stage B2 MMVD. An echocardiogram is recommended to both confirm the diagnosis of myxomatous mitral valve disease (MMVD) and to make measurements appropriate to stage this dog as Stage B2 with certainty. A proven diagnosis of Stage B2 MMVD would lead to recommendation to initiate pimobendan therapy in this patient (according to the 2019 ACVIM consensus statement). Alternatively, thoracic radiographs could be repeated in 6-8 months to identify the presence and severity of progressive cardiomegaly (increase in VHS of approximately 0.08/month or 0.5/6months). In the absence of an echocardiogram a VHS of 11.5, a VLAS of 3 or greater or evidence of progressive cardiomegaly as defined can be used to establish a diagnosis of Stage B2 and lead to the recommendation for initiation of pimobendan.
Clinical History
Signalment: 10 year old FS Chihuahua
Clinical history: Cinder was presented for evaluation of a heart murmur heard on routine physical examination. Cinder was bright, alert and grouchy on physical examination. Her heart rate was 120 bpm with a regular rhythm, strong femoral pulses and no pulse deficits. A grade 4/6 systolic murmur was heart best over the left heart apex. A grade 3/6 systolic murmur was also identified at the right apex.
