Radiographic interpretation:

Lateral radiograph: Cardiomegaly is present with a VHS of 11.8. The radiograph suggests right ventricular enlargement with increased sternal contract and lifting of the cardiac apex off the sternum. The carina is deviated dorsally; this could suggest either right or left ventricular enlargement. There is no evidence of left atrial enlargement (e.g. no loss of the caudal cardiac waist). The cranial cardiac silhouette seems enlarged and may be secondary to right atrial enlargement, main pulmonary arterial enlargement or aortic root enlargement.

Dorsoventral radiograph: The radiograph suggests right ventricular enlargement with a “reverse D” appearance. There is no evidence of left atrial enlargement. There is enlargement of both the aortic arch/descending aorta and the main pulmonary artery.

Radiographic diagnosis: Right ventricular enlargement, aortic arch/descending aorta enlargement, main pulmonary arterial enlargement, possible distension of the caudal vena cava.

Additional information: Echocardiography revealed a left to right shunting patent ductus arteriosus and severe valvular pulmonic stenosis.

Signalment: 8 month old M Rat Terrier

Clinical history: Joeyis reportedly asymptomatic at home. He has been owned since 12 weeks of age and a grade V/VI left basilar murmur was detected on his first and all subsequent veterinary visits. Thoracic radiographs were performed prior to castration.
